Case Management

Case managers are healthcare professionals who serve as patient advocates to support, guide and coordinate care for patients, families and caregivers as they navigate their health and wellness journeys.

Philosophy of Case Management
The underlying premise of case management is based in the fact that when an individual reaches the optimum level of wellness and functional capability, everyone benefits: the individuals being served, their support systems, the health care delivery systems and the various reimbursement sources.

​Case management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The case manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for both the client and the reimbursement source. 

Peer Supports

Certified Peer Support Specialists (CPSS) have experienced a mental illness and recovery and are well positioned to support others who are seeking recovery. Peers offer hope and encouragement by sharing their experiences and knowledge. They create opportunities for recovering individuals to live satisfying and meaningful lives.

​Certified Peer Support Specialists provide hope for individuals, families, and communities recovering from mental illness by offering:

  • ​Connections to a community of peers
  • Encouragement and understanding
  • Information on accessing resources
  • Support through recovery

 

Youth Supports

Certified Youth Supports assist and support the member in understanding their role in accessing services and becoming informed consumers of services and self-advocacy. Youth supports may include but is not limited to, mentoring, advocating, and educating through Youth Support activites indivdiually or in groups. Youth support services are provided in a context that is youth-cetnered, family-focused, youth-guided, strengths-based, team-based, community-based, out-come based, culturally sensitive and responsive to each youth's psychosocial, developmental and treatment care needs. 

​Youth support is intended for youth who have the capacity and ability to understand their diagnosis, needs, strengths, behaviors, and symptoms to be an active participant in making decisions for their individualized care. 

Community Based Rehabilitative Services

Skill Building/Community Based Rehabilitative Services (CBRS) is a home or community-based service that utilizes psychiatric rehabilitation interventions focusing on behavioral, social, communication, rehabilitation, and/or basic living skills training. The service is designed to build and reinforce functional skills and confidence. The goal is to improve the person’s abilities and confidence for successful independent living.
